We ask all clients complete a health screening/new client form.  The information you share is very useful for the teacher of your class and we need to ask for this for insurance and best practice.​

The paper forms are filed in a secure place for six months and are then destroyed.  Forms completed via Mindbody are stored securely on the Santosha Studio personal site.
